Forms for Faculty and Staff

For Department Use

All forms should be filled out and signed before submitting to AARO. Incomplete forms may be returned before processing is finalized, see “Form Procedures” for each form. All completed forms go to the email address listed for processing. Option to use Adobe-Sign for submission.

Course Registration Approval Form

This form is used by Department Chairs or Program Managers to approve student registration into a course. Approvals may include prerequisite waivers, course overloads, credit restrictions and junior status hold registration permission.

Form procedure: Submitted and approved by the Department Chair or Program Manager only > processed by AARO staff (aaro@collegeforcreativestudies.edu). 

Curriculum Change

Department Chairs and Program Managers should use this form to change a course requirement listed in the curriculum for an individual student. Curriculum Change forms are only processed after the approved course has been registered and/or completed. 

Form procedure: This form is approved by the Department Chair or Program Manager > processed by Assistant Registrar/Janine Surma (jsurma@collegeforcreativestudies.edu). 

Docusign Login

Login to complete or sign a document in Docusign.

Grade Change (request through AARO only)

This form should be used when changing a final grade that was given to a student. See “Grading Reminders” for more details on Grade Changes. 

Form procedure: Within 60 days of original recorded grade: Completed by Faculty > approved by Department Chair > processed by Assistant Registrar/Janine Surma (jsurma@collegeforcreativestudies.edu).

After 60 days of original recorded grade: Completed by Faculty > approved by Department Chair > Final Approval by Academic Affairs/Dean > processed by Assistant Registrar (Janine Surma)

Incomplete Grade <link to fillable pdf form>

The Incomplete form should be used if the faculty wishes to allow a student to turn in missing assignments the following semester (due to extenuating circumstances). An “I” grade is a temporary final grade. See “Grading Reminders’ for details on Incomplete Grades. 

Form procedure: Completed and signed by Faculty > Student Signature required > Approved/signed by Department Chair > processed by Assistant Registrar/Janine Surma (jsurma@collegeforcreativestudies.edu).

Incomplete Grade – Extension <link to fillable pdf form>

This form should be used  if the faculty intends to grant an extension for an “I” grade beyond the due date indicated on the original Incomplete Verification Form.

Scheduled Course Change Form

This form is used to make changes to scheduled courses. These changes include, faculty changes, classroom changes, day and time changes, course capacity, course cancellations, etc. 

Form procedure: This form is completed and signed by Program Managers > approved by Academic Affairs > processed by Assistant Registrar/Janine Surma (jsurma@collegeforcreativestudies.edu).
