Distinguished Alumni Award Nomination Form – Institutional Advancement

The College for Creative Studies’ Distinguished Alumni Award seeks to recognize CCS graduates who best exemplify what it means to be a creative leader. Alumni who have exhibited outstanding professional or artistic achievements, exceptional leadership in their community, a dedication to service, and a commitment to the advancement of artistic education should be considered for this award.

Nominees must be graduates of CCS and show a dedication to using their creative talents to create a positive impact in their personal and professional communities. Nominees must not have won the award previously.
