Skip to content
- Log in to DocuSign, navigate to the Ft and PT position request form under your Templates section, and click Use.
Note: If you do not see this form in your DocuSign account, contact Lisa Poszywak in Human Resources
- Under Requester, add your name and email address, then complete the name and email addresses of your Department Director / Chair as well as your Division VP and Click Send.
Note: the remaining signatures have already been assigned. Please do not add/delete any of the pre-filled signatures.
- Click Send, then Sign Now
- Complete all of the required fields, then select Finish
- At this point, you can login to your DocuSign account to see who needs to sign off on the form next. Once the document has been completely signed off on, you will receive an email notification from DocuSign.
