Use this form to request the creation of a new certificate, major, program, emphasis, or minor.
If the proposed path includes any new courses, a Request for New Course form must be submitted for each course. If the proposal involves discontinuing a course (i.e., it will no longer be offered), an Existing Course Change form must be submitted to indicate which course should be end-dated.
Effective Start Term: All new academic programs must begin in a Fall semester. Proposals should be submitted with sufficient lead time to complete all required institutional and external approval processes prior to implementation.
