Use this form to request a new certificate, major, program, emphasis, or minor. If the proposed path includes new courses, a Request for New Course form must be submitted for each. If the proposed path requires end dating a course (never offering it again), an Existing Course Change Form indicating which course to end date should be submitted.
PLEASE NOTE: Review the most recent edition of the NASAD Handbook, specifically Sections VIII and IX (undergraduate) and Section X (graduate), to ensure the titles and content of any proposed path align with NASAD criteria.
Note: Prepare well in advance. New programs must begin effective of the fall semester of the next available academic year. NASAD reviews new programs twice a year, October (1st deadline) and April (2nd deadline).
