Form – Request for New Program

Use this form to request the creation of a new certificate, major, program, emphasis, or minor.

If the proposed path includes any new courses, a Request for New Course form must be submitted for each course. If the proposal involves discontinuing a course (i.e., it will no longer be offered), an Existing Course Change form must be submitted to indicate which course should be end-dated.

Note: Please plan well in advance. All new programs must begin effective with the fall semester of the next available academic year.
