Use this form to propose a new course within an existing or proposed academic program, or to convert a Special Project course into a regularly offered course.
This form is required in conjunction with the New Program Request when proposing a new academic program or the Change to Existing Program form when adding a new course to an existing program.
A complete course syllabus must accompany this form and should include, at a minimum, the course description (or overview, if applicable), course learning outcomes, weekly schedule and assignments, grading assessment, and any other required syllabus components.
