An instructor may submit an Academic Alert Referral for any student in order to address an academic concern.<\/li><li><strong><a href=\"https:\/\/ccsedu.wufoo.com\/forms\/registration-permission-form\/\" data-type=\"URL\" data-id=\"https:\/\/ccsedu.wufoo.com\/forms\/registration-permission-form\/\">Registration Permission<\/a><\/strong> \u2013 An online form that requires the signature of the Department Chair or Program Manager, granting a student permission to register for a course that is filled or that has some other restriction.<\/li><li><strong><a href=\"https:\/\/ccsedu.wufoo.com\/forms\/course-withdrawal-form\/\" data-type=\"URL\" data-id=\"https:\/\/ccsedu.wufoo.com\/forms\/course-withdrawal-form\/\">Drop Request<\/a><\/strong> \u2013 An online form that the student must complete in order to begin the process of dropping (withdrawing) a course. The student\u2019s Academic Advisor must sign the form for drops. If the student is changing section numbers no Advisor\u2019s signature is needed.<\/li><li><strong>Curriculum Change<\/strong> \u2013 An online form completed by a Department Chair advising Registration to alter a student\u2019s curriculum in some way. Contact the <a href=\"mailto:aaro@collegeforcreativestudies.edu\" data-type=\"mailto\" data-id=\"mailto:aaro@collegeforcreativestudies.edu\">Academic Advising and Registration Office<\/a> to obtain the appropriate form.<\/li><li><strong><a rel=\"noreferrer noopener\" href=\"https:\/\/campus.collegeforcreativestudies.edu\/academic-advising\/wp-content\/uploads\/sites\/9\/2019\/10\/2022-09-14-AARO-CHANGE-OF-MAJOR.pdf\" data-type=\"URL\" data-id=\"https:\/\/campus.collegeforcreativestudies.edu\/academic-advising\/wp-content\/uploads\/sites\/9\/2019\/10\/2022-09-14-AARO-CHANGE-OF-MAJOR.pdf\" target=\"_blank\">Change of Major<\/a><\/strong> \u2013 A form with which a student may transfer from one program to another. The student must present their portfolio to the proposed new Department Chair for acceptance. The new Department Chair will sign and date the form, which indicates acceptance of the student into the major and confirms the assignment of studio credit.<\/li><li><strong>Declaration of Minor<\/strong> \u2013 A form with which an undergraduate student may declare a minor.<ul><li><a rel=\"noreferrer noopener\" href=\"http:\/\/campus.collegeforcreativestudies.edu\/academic-advising\/wp-content\/uploads\/sites\/9\/2020\/02\/AARO_LIBERAL-ARTS-MINOR-DECLARATION-FORM-22-23.pdf\" data-type=\"URL\" data-id=\"http:\/\/campus.collegeforcreativestudies.edu\/academic-advising\/wp-content\/uploads\/sites\/9\/2020\/02\/AARO_LIBERAL-ARTS-MINOR-DECLARATION-FORM-22-23.pdf\" target=\"_blank\">Liberal Arts Minor Declaration<\/a><\/li><li><a rel=\"noreferrer noopener\" href=\"https:\/\/campus.collegeforcreativestudies.edu\/academic-advising\/wp-content\/uploads\/sites\/9\/2021\/09\/AARO-STUDIO-MINOR-DECLARATION-FORM-22-23.pdf\" data-type=\"URL\" data-id=\"https:\/\/campus.collegeforcreativestudies.edu\/academic-advising\/wp-content\/uploads\/sites\/9\/2021\/09\/AARO-STUDIO-MINOR-DECLARATION-FORM-22-23.pdf\" target=\"_blank\">Studio Minor Declaration<\/a><\/li><\/ul><\/li><li><strong><a rel=\"noreferrer noopener\" href=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSePrLcNpiNmpE8YpzhkA-ZwF9tQeswwoOLfS8F8ddmMULC0bg\/viewform\" data-type=\"URL\" data-id=\"https:\/\/docs.google.com\/forms\/d\/e\/1FAIpQLSePrLcNpiNmpE8YpzhkA-ZwF9tQeswwoOLfS8F8ddmMULC0bg\/viewform\" target=\"_blank\">Complete Withdrawal Form<\/a><\/strong> \u2013 If a student intends a complete withdrawal from CCS for the semester they must meet with an Advisor in the Academic Advising and Registration Office and then a Financial Aid officer for an exit interview.
