{"id":1309,"date":"2021-06-07T18:22:00","date_gmt":"2021-06-07T22:22:00","guid":{"rendered":"https:\/\/campus.collegeforcreativestudies.edu\/academic-affairs\/?page_id=1309"},"modified":"2026-07-17T11:02:52","modified_gmt":"2026-07-17T15:02:52","slug":"form-development-request","status":"publish","type":"page","link":"https:\/\/campus.collegeforcreativestudies.edu\/academic-affairs\/form-development-request\/","title":{"rendered":"Form &#8211; Development Request"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Use this form to request funding or approval for a faculty, staff, program manager, or visiting artist professional development opportunity within the Academic Affairs division.<br><br>Please note the following requirements:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Include a two- to three-paragraph rationale describing how the proposed professional development opportunity will benefit both you and the College. This rationale must be attached within the form.<\/li>\n\n\n\n<li>Faculty requests that require time away from teaching must include a detailed plan for course coverage within the form.<\/li>\n\n\n\n<li>If the total cost exceeds the available Academic Affairs funding, an alternate budget center must be identified or the individual will be responsible for the remaining expenses. Academic Affairs will reimburse <strong>approved <\/strong>expenses only,<strong> <\/strong>and itemized receipts are required for reimbursement.<\/li>\n\n\n\n<li>If approved, the requesting department or form originator is responsible for making all travel arrangements and submitting the necessary payment requisitions.<\/li>\n\n\n\n<li>All reimbursement requests must be submitted within <strong>60 days<\/strong> of the conclusion of the event.<\/li>\n\n\n\n<li>If the event or travel plans are canceled, please notify the Office of Academic Affairs as soon as possible.<\/li>\n<\/ul>\n\n\n<p><b>You must log in to view this form.
