The FERPA policy is explained in the CCS Policy Database available to students.<\/p>\n\n\n\n<p><strong>This authorization form must be submitted by the student, otherwise it will be recorded as invalid\/voided.<\/strong><\/p>\n\n\n\n<p><strong>Log in with your CCS credentials to submit this form to the Academic Advising &amp; Registration Office (AARO).  Additional information may be required.<\/strong><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">STUDENT DIRECTORY INFORMATION<\/h2>\n\n\n\n<p>CCS defines the following as directory information:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li> Student\u2019s name <\/li><li>Dates of attendance <\/li><li>Graduation date <\/li><li>Major\/Academic Programs<\/li><li>Degrees, honors, and awards received<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">WITHHOLDING DIRECTORY INFORMATION<\/h2>\n\n\n\n<p>It is important that students understand the consequences of withholding directory information:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>If you request that this information be withheld, your directory information will be available only to college faculty and staff on a \u201cneed to know\u201d basis.<\/li><li>Any future request for directory information from non-institutional persons, organizations, other students, or relatives will be denied.<\/li><li>CCS\/AARO will not be able to verify enrollment or confirm a degree earned for possible employment, insurance purposes, galleries\/businesses, etc.<\/li><li>Withhold requests will remain effective until you cancel the request in writing to the AARO (including after graduation, leave of absence, etc).<\/li><\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Student Signature\/Request to Withhold<\/h2>\n\n\n<p><b>You must log in to view this form.
