The instructor must submit this form in addition to posting an “Incomplete” or “I” Grade on WebAdvisor.
IT IS THE RESPONSIBILITY OF THE STUDENT to contact the instructor regarding the above assignment and complete and submit that work to the Instructor no later than the last day of the subsequent 15-week semester. If the work is not turned in, this “I” grade will revert to a grade of “F”.
IT IS THE RESPONSIBILITY OF THE FACULTY MEMBER to fill out a “Grade Change Form” and submit it to the Academic Advising & Registration Office upon receipt of the work mentioned above. A copy of the “Grade Change Form” will be mailed to the student upon receipt by the Academic Advising & Registration Office. This will be the student’s verification that the work was received and a grade recorded.
IT IS THE RESPONSIBILITY OF THE STUDENT to contact the faculty member if they do not receive verification of a grade change from the Academic Advising & Registration Office within two weeks of turning in their final work.
